Magnetic resonance imaging plays a significant role in the diagnosis and treatment of tumors; however, its application in guiding precision radiation therapy is still in starting phase. To standardize the clinical use of MRI-guided radiation therapy in China, under the organization and leadership of the National Cancer Center/ National Quality Control Center for Cancer and with the participation of multiple healthcare institutions, this guideline has been developed by taking into account the practical challenges faced in MRI-guided radiation therapy. It systematically outlines the basic equipment, technical requirements, and personnel qualifications necessary for implementing MRI-guided radiation therapy, emphasizes the differences between MRI simulation for treatment planning and diagnostic MRI scanning, and provides detailed explanations for common tumors including the tumors of brain, head and neck, chest, abdomen, pelvis, and bone metastases regarding strategies for acquiring multi-sequence MRI images, methods for target delineation, and criteria for efficacy evaluation. Additionally, this guideline addresses common issues and solutions related to registration of MRI and CT images and explores the future applications of artificial intelligence and radiomics in MRI-guided radiation therapy, offering practical guidance to enhance the precision and efficacy of MRI-guided tumor radiation therapy.收起
