Objective To compare the long-term follow-up outcomes of three non-surgical treatment modalities—concurrent chemoradiotherapy, induction chemotherapy followed by concurrent chemoradiotherapydndu+CCRT), and induction chemotherapy followed by radiotherapy alone — in patients with locally advanced hypopharyngeal squamous cell carcinoma, and to analyze the safety and efficacy of these three treatment approaches. Methods A retrospective cohort study was conducted. Clinical data of 136 patients with LA-HPSCC who received non-surgical treatments at Shandong Cancer Hospital from January 10, 2012, to September 24, 2022, were collected. Of these patients, 131 were male and 5 were female; their ages ranged from 43 to 80 years, with a median age of 61 years. The patients were stratified into three groups based on their treatment modalities: the CCRT group with 42 cases, the In-du+CCRT group with 33 cases, and the Indu+RT group with 61 cases. Observational indicators: patients' clinical data, imaging examination results, and pathological examination results. Clinical endpoints: overall survival, progression-free survival, and treatment-related adverse events. The PFS rates, OS rates, and incidence of AEs across the three groups were compared. PFS and OS rates were calculated using the Kaplan-Meier method, and survival curves were plotted. The log-rank test was used to detect differences between the curves. Results The 5-year PFS rates were 35.6%, 42.3%, and 19. 2% for the CCRT, Indu+CCRT, and Indu+RT groups, respectively ; the 5-year OS rates were 37.9%, 34.4%, and 15. 7% . The OS in the CCRT group was superior to that in the Indu + RT group , and the PFS in the Indu+CCRT group was higher than that in the Indu+RT group if =6. 275, P = 0. 012). However, there was no significant difference in 5-year OS and PFS between the CCRT and Indu+CCRT groups. In patients with N2-3 stage, the Indu+CCRT group had a higher PFS than the other two groups {-£ = 6. 220, P=0. 045). Univariate a-nalysis revealed that esophageal involvement at the cervical level and T staging were risk factors for PFS and OS, both P<0. 05. Multivariate analysis indicated that T staging was an independent risk factor for PFS . The incidence of acute AEs in the Indu+CCRT group was 97. 0%, which was higher than that in the CCRT group [76. 2% ] and the Indu+RT group [62. 3% ], P<0. 05. Conclusions T staging is an independent risk factor for PFS in patients with LA-HPSCC. The 5-year OS with CCRT and Indu + CCRT regimens is similar in LA-HPSCC patients, but the Indu + CCRT regimen is associated with a higher incidence of AEs. For elderly patients , it is recommended to prioritize the CCRT regimen; for patients with N2_3 stage, the Indu+ CCRT regimen is recommended to achieve better PFS.收起
