Objective: This paper aims to investigate the impact of perioperative Enhanced Recovery after Surgery protocols on the quality of recovery following liver transplantation in patients with hepatocellular carcinoma, and to evaluate its efficacy and safety. Methods: A total of 92 patients who received allogeneic orthotopic liver transplantation for hepatocellular carcinoma at Qianfoshan Hospital of Shandong Province from August 2020 to June 2024 were included in this study. According to the perioperative management protocol, the patients were divided into an ERAS group and a conventional care group . The following outcome measures were compared between the two groups: time to first flatus, time to first ambulation, ICU length of stay, postoperative complication rate, total hospital stay, and hospitalization costs. Results: Compared with the conventional group, the ERAS group demonstrated statistically significant reductions in time to first flatus [3.5 days vs 4.2 days, Z = −4.702, P = 9.214, P = 0.002) and the incidence of delirium between the two groups. Conclusion: Implementation of the ERAS pathway during the perioperative period is associated with significantly accelerated postoperative recovery, reduced incidence of complications, and lower medical costs in patients undergoing liver transplantation for HCC.
