Objective To investigate the early clinical outcomes of total knee arthroplasty assisted by a 3D-printed osteotomy guide plate with a personalized prosthesis. Objective To investigate the early clinical outcomes of total knee arthroplasty assisted by a 3D-printed osteotomy guide plate with a personalized prosthesis. Methods A retrospective analysis was conducted on patients who underwent TKA with a 3D printed osteotomy guide plate and a personalized prosthesis at Shandong Provincial Hospital Affiliated to Shandong First Medical University between December 2024 and September 2025. A total of 20 patients were included in the personalized prosthesis group. During the same period, 25 patients with osteoarthritis who underwent conventional TKA with a standard prosthesis were matched as the standard prosthesis group. In the personalized prosthesis group, there were 7 males and 13 females. The mean age was 68.25±5.69 years, and the body mass index was 25.64±5.45 kg/m. According to the Kellgren-Lawrence grading system, 9 cases were grade III and 11 cases were grade IV. In the standard prosthesis group, there were 8 males and 17 females. The mean age was 67.56±6.17 years, and the BMI was 26.46±5.64 kg/m. According to the K-L grading system, 10 cases were grade III and 15 cases were grade IV. The intraoperative prosthesis coverage rate was compared between the two groups. Postoperative X-ray images were used to evaluate prosthesis position. The following parameters were measured: the coronal hip-knee-ankle angle, the coronal femoral component angle, the coronal tibial component angle, the sagittal femoral component angle, the sagittal tibial component angle, and the patellar tilt angle. Clinical outcomes were assessed using knee range of motion , the Knee Society score , the Western Ontario and McMaster Universities Osteoarthritis Index , the visual analogue scale for pain, and the Feller Patellar score. Results The mean follow-up time was 4.75±2.07 months in the personalized prosthesis group and 5.28±1.81 months in the standard prosthesis group. The difference was not statistically significant . The coverage rates of the femoral mediolateral diameter, the anterior femoral condylar width, the tibial plateau mediolateral diameter, and the anteroposterior diameter of the medial tibial plateau were higher in the personalized prosthesis group than in the standard prosthesis group. The differences were statistically significant . Similarly, there were no significant differences in WOMAC scores between the two groups at 1 month or at the final follow-up . The postoperative patellar tilt angle was 1.47°±0.50° in the personalized prosthesis group and 3.96°±2.22° in the standard prosthesis group. The Feller patellar score was 20.05±1.93 and 17.64±1.93, respectively. Both differences were statistically significant . Conclusion Compared with conventional TKA using standard prostheses, TKA assisted by 3D-printed osteotomy guide plate with personalized prosthesis achieves higher precision in bone resection, superior prosthetic coverage, leading to less early postoperative pain symptoms, and better patellofemoral joint function.收起
