Objective To summarize the current diagnosis and treatment status and latest progress of epidermal growth factor receptor gene exon 20 insertion mutation in advanced non-small cell lung cancer , provide guidance for clinical practice. Methods The keywords 'Non-small cell lung cancer, NSCLC, EGFR exon 20ins mutation' were used to search the PubMed. Clinical Trials, gov databases. Chinese national knowledge infrastructure , Wanfang database and Chinese Scientific Journals Database from January 1, 2019 to December 31, 2024. Inclusion criteria: definition and molecular characterization of EGFR ex20ins mutation. clinical detection methods for EGFR ex20ins mutant advanced NSCLC. epidemiological characteristics of the mutant disease. current clinical treatment status and the latest treatment progress registered clinical trial research data. Exclusion criteria:low-quality and outdated data literature. Finally, 66 articles were included. Results EGFR ex20ins represent a rare but highly heterogeneous mutation type in NSCLC, accounting for about 11% of all EGFR-mutated NSCLC cases. In contrast to other common EGFR mutation subtypes, such as exon 19 deletion mutation and exon 21 L858R point mutation , EGFR ex'20ins is less sensitive to EGFR-TKIs analogs. The specific spatial structural changes of EGFR ex20ins not only reduce the binding efficiency between EGFR ex20ins and traditional EGFR tyrosine kinase inhibitors , leading to poor response of EGFR ex20ins NSCLC patients to EGFR-TKI drugs, but also cause EGFR ex20ins mutants to be closer in conformation to wild-type EGFR, thereby increasing the difficulty of drug screening. Conclusion Although the treatment of EGFR ex20ins- mutant NSCLC still faces significant challenges, the in-depth exploration of novel therapies would provide more effective treatment options in the future.收起
