Objective To exploring the occurrence of pseudo-acute kidney injury induced by cyclin-dependent kinase 4/6 inhibitors and its differentiation from true-AKI . Methods A retrospective cohort study design was adopted, and breast cancer patients who received CDK4/6 inhibitors treatment at the Shandong Provincial Hospital Affiliated to Shandong First Medical University from January 1, 2023, to December 31, 2024, were collected. Patients whose serum creatinine increased≥ 1.5 times the baseline value or the absolute value of the increase ≥ 26.5 μmol/L after treatment were included in the study. Patients′ demographics, disease information, oncological treatment details, and renal function indicators before and after treatment were extracted. The estimated glomerular filtration rate was calculated based on the serum creatinine and cystatin C levels before and after treatment. The eGFR calculated based on SCr and CysC were denoted as eGFR and eGFR, respectively. Based on eGFR and eGFR, patients with elevated SCr were diagnosed for kidney injury and divided into P-AKI group and T-AKI group. The occurrence of P-AKI and T-AKI during CDK4/6 inhibitors treatment was analyzed by descriptive statistics, and the clinical characteristics of the two groups were compared. Results Among 174 patients treated with CDK4/6 inhibitors, 64 had elevated SCr, and the incidence of elevated SCr was 36.78%. All 64 patients were female, aged 33-75 years, with an average age of 52 years. Among them, 58 cases treated with abemaciclib, 4 cases with dalpiciclib, and 2 cases with ribociclib. The median follow-up duration was 12.9 months. Among the 64 patients, 36 were diagnosed with P-AKI, while the remaining 28 were diagnosed with T-AKI. The incidence rates of P-AKI and T-AKI were 20.69% and 16.09% , respectively. There were no significant differences between the two groups in the diagnosis/discovery time of SCr elevation, the increase degree of SCr and blood urea nitrogen, and the degree of eGFR decrease after treatment . However, the CysC level in the P-AKI group was significantly lower than that in the T-AKI group [ mg/L vs. mg/L], and eGFR was significantly higher than that in the T-AKI group [ ml/ vs. ml/], with statistically significant differences ml/ vs. ml/, P, as well as the difference between eGFR and eGFR.收起
