Radiation-induced hypopituitarism is a serious and common complication in patients with brain and head and neck neoplasms following radiotherapy, significantly impacting their long-term quality of life. Radiation-induced hypopituitarism is a serious and common complication in patients with brain and head and neck neoplasms following radiotherapy, significantly impacting their long-term quality of life. Recently, with advancements in radiotherapy technology, the pathogenesis, risk factors, and preventive measures for RIH have become increasingly clear. Radiotherapy-induced pituitary injury primarily manifests as abnormalities in the hypothalamic-pituitary axes. The growth hormone axis is most frequently affected, followed by the gonadotropin, adrenocorticotropic hormone, and thyrotropin axes. Radiation dose, irradiation volume, age, follow-up duration, and radiotherapy modality are key influencing factors. This article reviews recent research on RIH, including the latest advancements in the pathogenesis, risk factors, dosimetric parameters, clinical diagnosis and treatment, as well as long-term follow-up of RIH, to provide evidence-based references for clinical prevention and treatment, and offer insights for future research.收起
