AIM The aim of this study was to determine the accuracy of two-dimensional shear wave elastography combined with portal haemodynamics in the noninvasive assessment of the extent of liver fibrosis in patients with autoimmune liver disease , using liver biopsy as the reference standard. AIM The aim of this study was to determine the accuracy of two-dimensional shear wave elastography combined with portal haemodynamics in the noninvasive assessment of the extent of liver fibrosis in patients with autoimmune liver disease , using liver biopsy as the reference standard. MATERIALS AND METHODS This study included patients diagnosed with AILD via liver biopsy at a tertiary hospital. We retrospectively collected clinical baseline data, including portal vein flow velocity and diameter measured by abdominal ultrasound, as well as liver stiffness values measured by 2D-SWE. Univariate logistic regression and stepwise regression methods were used to screen for relevant variables. Receiver operating characteristic curves were constructed for the liver stiffness measurement model and the LSM plus portal flow velocity model, and the areas under the ROC curve of the two models were compared. RESULTS A total of 164 patients with AILD were included in the study. 2D-SWE combined with portal haemodynamic modelling predicted the AUCs to be 0.90, 0.92, and 0.94 for significant fibrosis, severe fibrosis, and cirrhosis, respectively. The area under the ROC curve for the 2D-SWE model in predicting significant fibrosis, severe fibrosis, and cirrhosis was 0.85, 0.87, and 0.88, respectively. The difference in the AUC between the LSM model and the LSM + PFV model in predicting significant fibrosis, severe fibrosis, and cirrhosis in AILD patients was statistically significant . CONCLUSION The model of 2D-SWE combined with portal haemodynamics showed good performance in assessing the stage of liver fibrosis in patients with AILD.收起
